How to file a case with Uprise


1. Select the “Help” option from the file menu

 



2. Select the “Submit a Support Ticket” button in the bottom left corner




Please be sure to provide specific details so that the Uprise support team can readily assist you.

  • Description- Provide a description of the issue within Uprise.
    • Ex: Unable to submit contact lens order

 

  • Reproduction Steps- Provide a step by step detailed explanation of how you got to the issue in Uprise. Include screens visited, buttons clicked, etc.
    • Ex: Selected patient, Clicked on RX Order, Clicked on Standard Contacts, Filled in Standard contact order with Acuvue Oasys 12 pk, Clicked Submit Order, Received a pop-up saying unknown error preventing order submission

 

  • Actual Results- Provide an explanation of the actual results the system produced.
    • Ex: Resulted in unknown error message when trying to submit order

 

  • Expected Results- Provide an explanation of the results you expected in the system based on your reproduction steps.
    • Ex: Expected the order to be submitted successfully and to receive an e-order number

 

  • Attach Screenshots- Add screenshots of error messages, invalid data, or anything else that is helpful in conveying the issue you are experiencing.
    • Ex: Attach a screenshot of the error code received when trying to submit the contact lens order
